MD24 House Call is a network of Physicians, Specialists, Nurse Practitioners and Physician Assistants that provide medical services to residents in senior retirement communities, and those with multiple chronic diseases. By leveraging our wearable mobile technology platform with physician-centric designed TeleMedicine, iHealth, and Electronic Healthcare Records, we bring the clinicians in all specialties to the patients in assisted, independent, group home, memory care communities, single homes, Chronic Care Management (CCM) centers, or wherever the patients are. Working in partnership with national laboratories, primary care clinician network, home health, hospice agencies, DME, pharmacies and specialists, MD24 House Call brings comprehensive on-site coordinated care to residents; alleviating the stress and hassle of transportation and wait time issues often involved with doctor visits. Our continued success in reducing unwanted trips to the hospital and preventing unnecessary re-admissions is ensured by our commitment to the continuity of quality care through regular visits. Now, we are excited to announce that MD24 House Call has launched an innovative telehealth program in multiple states in addition to our traditional "in-person" House Call services to enhance the same-day medical services at home, at no additional costs to the patients. Through this new program, we combine both TeleMedicine and "in-person" visits for all patients with multiple chronic diseases, anywhere and anytime. We empower the patients and caregivers with a smart phones, mobile tablets and support which help instantly connect the patients, caregivers and home care services with their own primary care physicians while our clinicians are serving our patients in their homes, custodial facilities or wherever the patients are. If qualified for our grant, we also provide the smart phone and hardware along with supports at no charge to strategic partners or patients.

To bring more values to our patients, we are now offering our innovative, patent-pending physician-centric TeleMedicine and iHealth platform to all primary care physicians, specialists, hospitals and home care services.
